Dental Insurance

Our Philosophy: 

We understand that patients rely on their dental insurance plan to help defray the costs of dental care.  We are happy to assist you in obtaining maximum insurance benefits.  We will help you to understand the provisions of your policy, and may accept payment directly from your insurance company for the portion covered.

What to expect: 

On the date of your office visit, you are responsible for your deductible and the portion we estimate your insurance policy will not cover. We will bill your insurance company within 24 hours of the service rendered.  Since the dental office is not the purchaser of the insurance policy, we have no leverage with your insurance company to obtain payment.  Our follow up is to re-bill your insurance company, and to supply any additional x-rays or narrative information requested.  If your insurance company payment is not received within 45 days of the date of service, the entire balance is due from you and reimbursement can be obtained directly from your insurance company.  As the purchaser of your dental insurance policy, you, the patient, have the most leverage with your insurance company to obtain payment. 

While we encourage you to make your dental health choices with the understanding that your insurance benefits provide you with a small financial cushion, we understand that many patients rely on dental insurance. That’s why we try to always have one of our dentists “in network” with many insurance companies.

For the convenience of our patients with dental insurance, Dr. Zukosky is "in network" with many insurance companies. In order to avoid compromises in your dental health choices, Dr. Gotthelf is out of network with most insurance companies.

Our courtesy service to you:

  • Filing your insurance within 24 hours of your visit.
  • Providing all necessary x-rays and documentation to the patient and to the insurance company.
  • Researching your dental insurance plan to advise you of benefits available to you.
  • Re-filing your insurance a second time within 30 days.
  • Following ADA guidelines for dental procedure codes and insurance submission.

Your responsibility as owner of the policy:

  • Payment of fees not covered by your insurance plan at the time the service is delivered.
  • Acknowledgement that the dental insurance policy belongs to you; the dental office has no leverage to obtain payment from your insurance carrier.
  • Acknowledgement that based on the policy purchased, dental insurance plans restrict payment for some services, use restricted fee schedules (called Usual and Customary Rates), and exclude some procedures based on prior conditions or length of time on the plan.
  • To take responsibility for payment if your insurance company does not pay our office within 45 days.
  • Keeping our office informed about any changes in coverage or employment.
